Use this form to request access to GP online services on behalf of another person.

You can use this service if:

• the patient is registered at this GP surgery
• you are authorised to act on the patient’s behalf (for example, as a parent, carer, or legal representative)

Before you start

We’ll ask you for:

• details of the patient you are requesting proxy access for
• your first and last name, date of birth, sex, postcode, email and phone number
• which online services you are requesting access to
• proof of identity
